New Patient Information & Policies
FEES FOR CASH PATIENTS:
First time office visits can take up to one hour, and the fee is $275
(You must pay half of this fee- $137.5 - prior to the visit and the other half after the visit.)
Future or return visits take approximately 30 minutes, and the fee is $135.
The fee for return visits that take less than 15-20 minutes is $85.
Our biller will charge you a late payment fee of $5 every time you are late on a bill.
Note: Often Dr. Sharif offers a homeopathic remedy to his patients during the visit. The charge for this remedy is
$15 which is not reimbursable by insurance companies.
INSURANCE: Note: Since insurance companies can be slow in processing the claims, it might be weeks before you
receive a bill from our office for your portion of the bill. It is YOUR responsibility to know if your insurance pays for
naturopathic medical visits, and how much they will pay towards each visit. We are only billing your insurance as a
courtesy to you.
We are currently contracted primary care providers (PCPs) for Regence, Premera, Lifewise, Aetna, First Choice, Molina
Marketplace, Kaiser Permanente (that has First Choice Health printed on the card). The state insurances that we are
contracted with are Molina and United Healthcare Community Plan. We are not contracted with Cigna or Kaiser
Permanente CORE plan at this time.
If your insurance carrier is not contracted with the doctor, then you will be expected to pay our office fees in cash at the
time of service. We can provide a super bill for you at time of service which you can submit to your insurance company
to request reimbursement if you have out of network coverage. You can call your insurance company and ask them the
extent of their âout-of-network coverageâ for first office calls, return visits, as well as laboratory fees.
DISPENSARY ITEMS:
We have a dispensary at our office, and many of the supplements/remedies that are prescribed to you can be obtained
from this dispensary. Unfortunately, at this point in time, Natural medicines (âsupplementsâ) are NOT covered by
insurance companies, and you are expected to pay for them yourself. Note that we do not accept returned
medicines/supplements. We do NOT charge taxes on our supplements for our current patients.
We are obliged by law to charge taxes on supplements for patients that have not been in for more than a year.
PAYMENT:
Payment is due at the time of service. We accept cash, check or credit cards.
CANCELLATION / NO SHOW Policy: Due to the nature of our naturopathic/homeopathic visits (which differ from
the rather short visits with conventional medical practitioners), we dedicate 30-60 minutes per visit specifically for you.
If you do not show up for your appointment or neglect to give us an advanced notice of at least 24 hours, you may be
charged a fee of $50.

FMLA (Paid Family and Medical Leave):We do NOT fill out FMLA forms for patients. If you need these forms filled
out, please seek help from other medical clinics.
